Please note : only use this form for comments about the practice and suggestions as to how we can improve our service to you.
Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.
Want to make a complaint?
We welcome your views on the service provided at the practice. All comments are considered at regular team meeting and we will endeavour to provide a response within 30 workings days.
If you have a complaint, please put your complaint in writing addressed to the Practice Manager or email practice.manager.w97021@wales.nhs.uk. You will receive an acknowledgement within 2 working days of receipt of your complaint. Your concern will be investigated and the findings will be sent to you within 30 working days. If the complaint takes longer to investigate, you will be notified of the expected timescale. Alternatively please use the complaints form link.
Alternatively, you can raise your concern with the Local Health Board.
