Hospital related Sick Notes
Responsibility for Issuing a Med3
If your request is related to a visit to any Hospital facility, it is the responsibility of the hospital to issue you with a Med3 for the duration of any anticipated period off work. The responsibility for issuing a Med3 rests with the doctor who at that time has clinical responsibility you.
Where a patient has been referred to a hospital for an opinion or advice on their health condition, responsibility for issuing a Med 3 should remain with the GP.
Sick Notes
Doctor’s Sick/Fit Note for more than 7 days
If you have been ill for more than 7 days you may need a doctor’s certificate. These certificates are called ‘Fitness to Work’ Certificates.
If you have not consulted a healthcare professional (i.e. common ailments pharmacist, physio, mental health nurse) about your illness, PLEASE DO NOT COMPLETE THIS FORM, as we cannot issue one. Instead, you will need to call reception for them to add you to our triage list, and it is at the GP's discretion on whether you can be issued a sick note.
Requesting an additional Sick/Fit Note
If you have already had a Sick Note (Fit Note) for this illness your Doctor may not need to see you to issue an additional Sick Note. Please complete this form. We will contact you to let you know when you can collect your Sick / Fit Note, or we may contact you to advise you need to arrange an appointment before we can issue another note.
